Also on the menu bar shadows and highlights need to darked more so the menu bar is more expressed.
Probably shrink menu bar height a couple of pixels so windows dont look top heavy.
I noticed that in the several versions this theme had a pinkish tint... is this a feature or bug? - Jun 08 2005

But with a couple issues below:
I found a bug in bevel_maximized has a brighter highlight than the bevel highlight. Or it could be the other way around.
I actually prefer the highlight in the bevel_maximized however because its more visible. It looks good and familiar to the Indubustrials/Industrial/Human/Glider highlight which are all awesome and many like.
Another issue I noticed is that to the top-right of the window, at around title_height+4 where the vertical gradient ends, when a focused window is against a dark background you notice a sharp cut off. Like missing pixels but its all there, its just too dark. It bleeds into the background. Solution would to remove the shadow/midtone right there. The right black vertical line is good enough.

I did some changes to the restore, minimize, maximize, and close buttons. I also changed the way the window looks when its maximized.
I also fixed a few glitches, missing pixels, missing lines, redundant black pixels and other more.
I believe that Smooth GNOME is a dervitive of the original novell industrial theme. You can find it named Glider on Gnome 2.8. I'd guess this because they look exactly the same.
